Another posible solution is 64:

(1 + 4 - 0) * 1 = 5
(2 + 5 - 1) * 2 = 12
(3 + 6 - 2) * 3 = 21
(8 + 11 - 3) * 4 = 64

Another way to get the 96 result (making the assumption that there are 5 missing operations)

(4° = 32, 5° = 45, 6° = 60, 7° = 77, 8° = 96)... incrementing 2 to the difference between results.

Of course you can get 96 by simply

1 + 1 * 4 = 5
2 + 2 * 5 = 12
3 + 3 * 6 = 21
8 + 8 * 11 = 96

I have done the calculation like this:

First number (first row) * second number (next row)

And to know the second number in the next row you take the second number in the first row and add 1 to it

1 * 5 (4+1) = 5
2 * 6 (5+1) = 12
...
8 * 12 (11+1) = 96

the last line is incorrect, it should be

1+4*(1)=5
2+5*(2)=12
3+6*(3)=21
4+7*(4) =32
5+8*(5)=45
6+9*(6)=60
7+10*(7)=77
8 +11*(8) =96

so the answer is still 96 :)
