Android Question Login form (example) + loading

Hi all

I'm looking for an example (source code) of a login "form" (2 textboxes + 1 button)
and a "full screen" loading animation.
Where can I find these things?

And how can I apply a template?
Actually I have a .CSS used into a website (textbox with shadow, button that "on mouse over" changes color, ....)
