Android Question Missing Launch Icon

I have written a very simple app, testing on my Samsung Galaxy Note via B4A Bridge. It now runs OK from an icon on the home page.
I put it on my granddaughter’s Lenovo TB3-710F (Android 5.0.1) via B4A Bridge. It runs fine, but there is one problem – no visible icon for the app.
I can go to Settings - Apps, find the app (which displays the correct icon against the App name) and launch it from there – no problem.I have tried uninstalling / reinstalling, but still no icon.
The icon is small, only 1kB.
I have restarted the Lenovo several times.

Note that the file size doesn't matter. Only the actual width and height.

What happens if you create a new project with a different package name (Ctrl + B) and keep the default icon. Does it appear?

I've created a completely new app with the deafault icon and uninstalled old app from Lenovo.
Installed the new app. The default icon appears in settings - apps, but no icon on home page.

Second attempt with same app worked. I have no idea what has changed!

Tried the proper app - its icon also showed up.

It sounds like an OS bug.
