Android Question Survey solution

I'm looking for a survey solution (forms with parameterized questions), do anyone here did a similar solution with B4A?

have a look on http://questionform.com/ for having an idea concerning the survey form

goal:

1. Creation of form with questions, could be a CSV...
2. Apk will download the survey
3. Survey entry on the device (tel or tablet)
4. responses of the survey go back (Main Db update with Web service)
5. Scoring of the main DB = Question 1, answer 1=1 point, answer 2=3 points, answer 3= 2 points) + Question 2 ....

Any ideas???
