Android Question Completing Ads declaration when submitting app to Play Store

I must be missing something very obvious but I just can't see how to complete the Ads declaration screen when submitting a new app to the Play Store. (I haven't gone through the process for over a year.) This is a screenshot of the relevant page. The two buttons are greyed out and there is nowhere to enter any info. I have tried in both Chrome and Firefox. I have sent a query to Google but I am not expecting a response for several days.

In fact Google replied very quickly and suggested my browser's ad blocker was the problem. And they were correct. Slightly ironic that the question about ads was blocked by an ad blocker.
