Do you document your projects?

By documenting, I mean, going beyond simple comments on your code.

I do take notes sometimes and I just started using flowcharts, it really helps!

I do not document my apps , just simple notes around functions blocks

I used to start an app by writing code directly. I just take note by using comment. I know it's bad because after a few months I can't remember what the hell I did

sure, it is very important to use comments in your code. it will make it easier for you to understand it after a while you worked on other projects.
