Android Question How can I convert B4J to B4A User Input Form?

I've watched Erel's Forms & User Input video tutorial using B4XPreferencesDialog and this is just what I need for my Android app.

Unfortunately, I have no idea how to convert the B4J example file for use with B4A.

Could someone please advise me how to go about doing this or point me in the direction of a tutorial.
